TMS Therapy: What to Expect During Treatment

Once someone decides to move forward with TMS therapy, a new set of questions usually comes up, less about whether it works and more about what the actual experience of treatment looks like day to day. Understanding what happens during a course of TMS can make the whole process feel far more manageable before you ever sit down for a first session.

Treatment typically begins with a mapping session, where your provider determines the precise location and intensity setting needed to stimulate the correct area of your brain. This first appointment usually takes a bit longer than subsequent sessions, since it establishes the settings that will be used throughout your treatment course. You will be awake and seated comfortably the entire time.

During an actual TMS session, a magnetic coil is placed against your scalp, positioned according to the mapping done in your first appointment. The device delivers repeated magnetic pulses in a specific pattern, which produces a tapping or clicking sensation and sound. Most patients describe this as odd at first but tolerable, and something they adjust to within the first few sessions.

Sessions generally run between twenty and forty minutes, and you remain fully alert throughout, with no sedation involved. Many patients use this time to relax, listen to music or a podcast, or simply rest. Afterward, there is no recovery time needed. You can drive yourself home, return to work, or go about the rest of your day exactly as planned.

A full course of TMS typically involves daily sessions on weekdays over several weeks, which is a real time commitment to build into your schedule. Consistency matters here, since the treatment works cumulatively, with each session building on the effects of previous ones rather than functioning as a single standalone intervention.

The most commonly reported side effect is mild scalp discomfort or a headache, particularly in the earlier sessions before your body adjusts. This tends to improve as treatment continues and is generally manageable with over-the-counter pain relief if needed. Serious side effects are rare, which is part of why TMS has become an appealing option for patients wary of the broader side effect profile of psychiatric medications.

Knowing what to expect before you start can make the entire process feel far less uncertain. If you are considering TMS therapy, our team can walk you through exactly what your treatment schedule and experience would look like based on your specific situation, so you are fully informed before making a decision.
